Returning Student-Athlete Packet Drug and Alcohol Policies and Information NCAA Banned Drug List '08-'09 Dear Eagle/Lady Eagle: I hope you are enjoying your summer break and are looking forward to playing for Carson-Newman this upcoming year. In order to provide the very best health care for you while at C-N, several forms have been developed. Please find them enclosed with this letter. I would request that you take a few minutes together with your parents/guardians and fill them out. Please return these forms to me as soon as possible. Taking care of this now will greatly simplify matters when you report to campus. You cannot participate in any athletic activity at Carson-Newman until all of this paperwork is completed and on file in the Athletic Training Room. All information requested will be kept in the strictest confidentiality. A separate letter to your parents regarding health insurance for you while participating in athletics at Carson-Newman is enclosed. Please work with your parents to ensure that the form enclosed with that letter is also sent back to the school. It is absolutely critical that this form be filled our COMPLETELY and ACCURATELY. If this form is not filled out completely and accurately, you will not be allowed to compete in athletics until it is. Further, failure to have this form filled out properly and on file in the athletic department office may cause you to have to pay for any medical bills you might incur while participating in athletics.
